Many individuals have health insurance plans that will reimburse a portion of your costs for OON treatments. Please complete the form below to authorize this practice to obtain your specific coverage & deductible information for you. After you verify that your insurance will reimburse you for the services you receive from L.I.G, you can also file a claim.
